It should be a godsend for one & all to sport our names, front & center. No more having to spell out, "D E E V" - it will be there for all to see, from the residence's new director to my oldest friend.

Off to an appointment? Take blanks for the care provider & support staff. Keep blank tags & a fine point Sharpie wherever you store your lanyard. You could fill them when you get there -or- call ahead so you can just quietly hand them over on arrival. (Most care providers have their name visible, sewn into a lab coat or on a name plaque, but it's often too small for older eyes to read.)

Be sure to get your loved one or care partner in on it, crafting his or her own name tag. You can include important information on the back, such as full name, home address, contact #, etc. BONUS: the sooner they get used to wearing it as a matter of course, the more likely they are to remember it later.
Takes me back over thirteen years, to when I worked at our area's first Panera Bakery-Cafe. Each new associate writes out & decorates a name tag. Customers enjoyed the ways we each showcased our name. Remembering the instruction we got - make sure the name is spelled clearly, is easy to read (writ LARGE), and looks like fun.
If you know someone with memory challenges, start using name tags! Lanyards are easiest to wear & can be easily stored in a handbag or briefcase, pocket or glove compartment. Keep blanks & a clear pen out in the open so everyone who stops by - to clean, give meds or for a social visit - can wear one.
